Dosing & Treatment Details

Testosterone Cypionate

The most widely prescribed TRT in the US. Long ester (8-day half-life) allows weekly or bi-weekly dosing. Typical starting dose: 100–200mg/week injected IM or SubQ. Dose is titrated based on labs targeting total T of 700–1000 ng/dL.

Testosterone Enanthate

Very similar to Cypionate with a 7.5-day half-life. Often preferred for those who notice differences in carrier oil. Same dosing approach — weekly or bi-weekly at 100–200mg/week.

HCG (Human Chorionic Gonadotropin)

Used alongside TRT to maintain testicular function, size, and fertility. Mimics LH signaling to stimulate endogenous testosterone production in the testes. Typical dose: 500–1000 IU 2–3x/week.

Clomid / Enclomiphene

SERM-based therapy for men who want to stimulate their own testosterone production without exogenous testosterone. Often used for younger patients, fertility-concerned patients, or those wanting a more conservative approach. Oral, daily dosing.

Do I have to inject forever?+
TRT is typically a long-term commitment since stopping will return testosterone to baseline. Some patients transition to HCG or Enclomiphene monotherapy. Your provider will discuss options at every check-in.
Will TRT affect my fertility?+
Exogenous testosterone suppresses sperm production. If fertility is a concern, HCG + TRT or Enclomiphene monotherapy are preferred options. Discuss this with your provider during consultation.
How often will I need labs?+
Baseline before starting, then at 6–8 weeks after starting, then quarterly once stable. We monitor testosterone, estradiol, hematocrit, PSA, and metabolic markers.
What if my estrogen gets too high?+
Some testosterone converts to estradiol via aromatase. If your labs show elevated E2 with symptoms (water retention, mood changes), we may add a low-dose Anastrozole. This is monitored carefully.
